This form will allow you to submit an application to enrol into the Joint Training Program in Paediatric Emergency Medicine with ACEM.

To enrol in the program, you must be either:

• registered as an Advanced trainee with ACEM, or
• a Fellow of ACEM (FACEM).

The requirements of the Joint Training Program in PEM are specified below.

If you believe you have satisfied a requirement by the end of the previous training year, in whole or in part, please provide the details of the relevant terms. You do not need to provide evidence for the terms you have completed.
