Once a month you will fill out the Monthly TAP Reporting Sheet and submit it to TTS. Once TTS receives the information, the following tasks will be performed by TTS by the end of the second week of the following month.

Settlement Statement Analysis

Track Revenue per Mile

Track Cost per Mile

Track Miles Driven

Track Per Diem - Monthly & YTD

Monthly Profit and Loss Statements

Estimated Quarterly Taxes and Related Fillings

Does Not Include Annual Tax Preparation Fees

I hereby authorize TTS to deduct from my credit card the monthly TAP fee for services listed above in accordance with the amounts in TTS' current pricing structure and according to the terms of this agreement.

As soon as we receive this form, we will call you to walk you through the monthly process, acquire Credit card information, and set dates to debit your account.
