  • As a Personal Income Tax Client, we need to collect the following information from you in order to begin to prepare your income tax return.  Please do not submit until you have everything received and ready.

     

     

    Important: We require one form per person per tax year for whom we are preparing taxes.  The taxpayer must submit their own information as there are places where they must digitally sign.

    • Pensions – Old Age Security T4A(OAS), Canada Pension T4AP, other pensions, and annuities T4A.

    Please ensure to gather to upload all income slips including but not limited to...

    • Childcare – with supporting receipts and SIN of the caregiver
    • Adoption costs
    • Tuition Receipts – T2202 (Note - no other receipts for school expenses are deductable)
    • RRSP Contributions (for Mar - Dec of the current tax year or 1st 60 days of the subsequent year)
    • Child or Spousal Support – with a copy of the agreement if not previously provided
    • Rent / Property Tax - Ontario Only
    • Professional Fees or Union Dues (not included on T4)
    • Medical Expenses - Worksheet required for more than 5 slips
    • Charitable Donations (not included on T4)
    • Interest on Student Loans
    • Political Contributions

    •       provide information, tax slips and receipts for your T1 General Income Tax and Benefit Return;

    •       information given on your T1 General Income Tax and Benefit Return;

    •       adjustments to your T1 General Income Tax and Benefit;

    •       information about your registered retirement savings plan, Home Buyers' Plan, tax-free savings account and Lifelong Learning Plan;

    •       your accounting information, including balances, payment on filing, and installments or transfers;

    •       information about your CRA account benefits and credits (Canada child benefit, goods, and services tax/harmonized sales tax credit, and working income tax benefit); and

    •       your marital status (but not information related to your spouse or common-law partner)

    •       change of address;

    ·        Access any information on your file from the moment we find out there is a divorce or separation that has occurred

    Your responsibilities

    It is your responsibility to monitor and understand the transactions your representative completes on your behalf, and to make sure that the information for your representative is current.  It is your responsibility to let us know immediately should your marital situation change to separated or divorced, or should you decide to retract the authorization.
