Notify W.T.K if any of the following occurs:
Change in Users address or residence.
Any serious injury, surgeries, illness, or death of a Service Dog.
Any sudden change in dog's behavior or physical looks.
Any accident resulting in damage or injury to persons, animals, or property caused by Dog.
The need to retire Service Dog.
Ownership of W.T.K Jacket:
The Service Dog Jacket (vest) represents W.T.K’s licensing authorization for the Service Dog User to use the Service Dog as a Service Dog. The issued Jacket remains the property of W.T.K and must be returned upon the death or retirement of Service Dog. W.T.K reserves the right to maintain use of the Jacket, User must fulfill a minimum of 8 annual hours of Service Dog training as well as completing a required Public Access test with a W.T.K approved trainer before reservice full ownership of Jacket. Other training is encouraged, but will not fulfill these requirements. Use of Jacket puts User in “Good Standing” status. If at any point the Dogs service career, this contract of continued education hours is breached, the opinion for retaining a W.T.K service jacket will be void. Options to renew this will be based upon board approval.
Use of gear:
W.T.K reserves the right to ask User to use the following gear at all times:
Program given Jacket (vest) OR specific gear that has the Dog labeled appropriately in public settings.
Any gear such as tools, mobility harnesses, ect MUST be cleared by W.T.K’s board. Further instructions will be given from there.
Leash, Per the ADA 2017 a dog shall only be off leash if their tasks or disability interfere. If the above is true and User chooses to work Dog off leash, W.T.K has no legal binding to the training, usage, and results of an off leash Service Dog.
Boots/Shoes must be worn if temperatures are above 80 F or below 20 F, or the usage of escalators. If proper measures are not taken for the safety of the Dog, W.T.K has the right to take Service Dog or take legal measures against User. Boots will not be provided upon Service Dog however will be required during the handling process.
Use of dye:
W.T.K allows one (1) of the following options to be dyed on a dog. Ears, tail, paws. W.T.K ONLY certifies the use of Opawz pet safe dye to be used appropriately by following the given directions on the bottle. Please make a trainer aware if dog is dyed so we can update the physical descrition on dogs charts. We ask handlers to do a test spot first to rule out allergy, skin irritation, ect.
Resolution of Disputes:
The Service Dog User is encouraged to contact W.T.K with any problems regarding gear, training, or questions/concerns. If a decision of W.T.K results in a dispute, User may appeal to the Board of Directors. If a dispute is not resolved, W.T.K and the User agree that all disputes concerning this Contract shall be resolved by arbitration conducted in accordance with the procedures set forth in Iowa Revised Statutes Sections 36.600 through 36.740. A copy of this code is available upon request.
Terms and conditions of Contact:
By completing this contact, User will-
Properly feed, shelter, and care for Dog in a kind and humane manner.
Maintain the Service Dog in a good physical condition, keeping them a reasonable and healthy weight.
Provide all reasonable and necessary veterinary care, and provide the Dog with an annual veterinary examination.
Not lend, give or sell the Service Dog to any other persons. The Service Dog User will not allow the Service Dog to be used by any other person, nor shall allow Dog to task routinely for another person.
Not lend, give or sell the Service Dog Jacket to any other persons.
Not use or handle the Service Dog in any manner that would put the health or safety of the Service Dog User, the Service Dog, nor the public at any risk.
Not use the Service Dog in any manner that would reflect poorly on the Service Dog User, the Service Dog, or W.T.K.
