A field trip is scheduled for our Kindergartners to vist Southern Belle Farms located at 1659 Turner Church Rd, McDonough, Ga 30252. Please keep the follwoing in mind:
- Trip Date: Tuesday, April 14th
- Trip Cost: $25.00 (per scholar)
- The group will depart at 9:00 AM and is estimated to return at 1:00 PM.
*DIA will not accept verbal approval*

**This form should be completed by the parent/guardian. Payment can be made in infinite campus by anyone**
I have read the information above and give my permission for my scholar to attend the enrichment field trip listed above. I understand that my child will be traveling to and from by bus. I also permit my child to receive the necessary medical treatment(s) in case of emergency
