    You are not able to electronically submit this form to the host team from this site.

    INSTRUCTIONS:  Enter info into the blanks.  The fees will auto calculate for you.

    When you have completed filling in the information, click on PREVIEW PDF button at the bottom of the form. 

    A new window shows your completed form.  In the upper right corner click on the down arrow button to download a copy of your form (in PDF format) to your computer for printing or email.  Then send your form, with any deposit, to the host club.  

    If you would like to complete another form, you can click on CLEAR FORM to remove all your data or simply go back into the form and overwrite any info that changes

  • Participating teams may request an entry reservation from the Meet Host. Entry reservations must be submitted on the current LSC Meet Reservation form and received by the entry deadline stated in the Meet Announcement. Reservations cannot be postmarked before:

    SEASON LSC TEAM NON-LSC TEAM
    Short Course (Fall/Winter) 2nd Tuesday of September 3rd Tuesday of September
    Long Course (Spring/Summer) 2nd Tuesday of March 3rd Tuesday of March

    To hold any entry reservation, a Meet Host may require a deposit not to exceed the actual entry fee. Any reservation deposit may not be cashed or deposited before the meet entry deadline specified in the sanctioned or approved Meet Announcement.

  • Enter the number of swims for each session and the individual swim fee. 

    The form will automatically calculate the session totals that appear on your form.  The final amounts are also automatically calculated in the boxes below.

  • Note: Relay, surcharges and time trial fees are not figured into the reservation total.

    Reservations in excess of actual entries are not refundable and may not be applied to relay, surcharge, or time trial fees, UNLESS OTHERWISE SPECIFIED ON HOST TEAM'S WAIVER.
