1. I am 18 years of age or older.
2. I will provide food, shelter, medical care, love and affection for the lifetime of this cat, which could be as long 20 years or more.
3. I agree to take this animal to a veterinarian at my own expense for periodic exams and vaccines, as needed, and to obtain veterinary care should this cat become ill or injured.
4. I am not adopting this cat to be an outdoor only cat.
5. I understand and accept that the Caregiver and Partners for Cats do not convey irrevocable ownership of the adopted cat to me. I cannot sell, transfer, trade, give away or lease the adopted cat to any other person or organization without first notifying the Caregiver and Partners for Cats.
6. I understand and accept that I am legally obligated to return the adopted cat to the original Caregiver or Partners for Cats if they should request such a return.
7. The Caregiver and Partners for Cats have my permission to inspect the premises under which the adopted cat is kept at any time to verify the cat’s welfare.
8. I understand and accept that the Caregiver and Partners for Cats may remove the adopted cat from my possession if they determine . . .
a. conditions are unsuitable for the safety, health or well-being of the adopted cat.
b. food, water and/or shelter provided for the adopted cat are inadequate.
c. I have falsified any information on this application.
and I understand and agree that I have no recourse of any kind resulting from such action by the Caregiver or Partners for Cats.
9. I understand that the Caregiver and Partners for Cats make no representations or warranties regarding the health, size or
temperament of the cat being adopted and that the Caregiver and Partners for Cats expressly declaims the same.
10. I agree to hold harmless the Caregiver and Partners for Cats, its officers, directors, agents and representatives for any and all claims, losses and damages, whether to persons or property, that may arise as a result of the adopted cat’s health condition or behavior.
11. I understand and accept that this application remains the property of Partners for Cats.
