SURCHARGES

An additional late night charge of $30 is applicable for swabs performed between 8pm to 8am daily.
An additional Public Holiday Surcharge of $50 is applicable for swabs performed on Public Holidays in Singapore.
An additional Island Surcharge (Changi Airport, Jurong Island, Sentosa, Tuas, Restricted/Prohibited Areas) is applicable for swabs performed at these specified locations.

Please note: All surcharges specified above are not included when making payment at the end of this order form. These charges will be due after your appointment slot has been confirmed by the Speedoc Team.

1. Speedoc will do our best to honour your selected appointment date & timing, but due to high demand, we may contact you to offer a different appointment time.

2. Patients are responsible for checking the testing window requirements of their specific event and if Speedoc can meet the required timeline.

3. I authorise Speedoc to submit my particulars to the authorities for travel documentation purposes.

4. I acknowledge that my particulars submitted are accurate.

5. By proceeding, I agree that Speedoc shall not be held responsible for any delay due to inaccuracies in my submission.
