UC College Credit Plus Modality Change Form

This for is ONLY for current UC CCP students who were admitted as "In High School" and only took UC courses offered in their high school building and now want to be considered for admission for classes on UC's campus or online, too. Please note, a new CCP application is NOT required. IF YOU WERE ADMITTED FOR ON CAMPUS/ONLINE OR BOTH ON CAMPUS/ONLINE AND HIGH SCHOOL, DO NOT SUBMIT THIS FORM.
This form must be completed according to our application deadlines. Therefore the deadlines to change modalities for future semesters are:

Summer 2024 deadline = April 1st

Fall 2024 deadline = June 1st

Spring 2025 deadline = November 1st
