How to disable a form?

I am trying to disable form https://form.jotform.com/60718255148155 that goes into our connect wise system but can not locate in the My Forms. Can you please help me?

Also please send answers to angela.beaz@udtonline.com as the person where the mailbox is set to is out on maternity leave.

You may find the questions asked on the support forum by clicking on My Questions button in the support forum.

The forum responses shall be sent to the email address registered with the account only.

The form that you have provided here is associated with a different account ebarreto. You may ask the concerned person to login into this account and disable the form.
