I have checked your form in mobile view, and all fields look nice in portrait:
Except the Autocompleted Address widget at the bottom of your form:

To fix this, please inject the following CSS code: http://www.jotform.com/help/117-How-to-Inject-Custom-CSS-Codes

Thanks for that Bdavid. That autocomplete shouldn't even be there! I must have added it in error when I was editing it, so I'm glad you've found it!
However, given that I've now deleted that particular autocomplete address widget, the original one on the first page still doesn't show as being the correct size on my own iphone.
Perhaps the iphone is using a cached form? I'll have another look at tomorrow and update this thread.

You are welcome! Looking forward to know you updates on this, if after clearing the cache you still see it not fitting correctly in portrait view, please let us know the exact iOS you have, also what browser you are using, so we can try to replicate the issue.
