Hi again,
Upon checking your forms, the "Teacher application" form does not have a "Sender Name" and recipient email address, which may be the cause of un-received email notifications.
While the second form "Materials application" has no notification set under "Emails".
Please check this guide on how to setup your email notifications: http://www.jotform.com/help/25-Setting-Up-Email-Notifications. Please do make sure that you fill out all the necessary field to avoid problems.
