This position includes the following initial employee benefits:
A. Paid vacation of 10 days after the first year of full time employment per company handbook.
B. Paid holidays after the first year for full time hourly and salaried employment per company handbook.
C. Eligibility to participate in the 401K plan after the first year of full time employment per company handbook.
D. Eligibility to receive pension profit sharing bonus after the first year of employment per company handbook.
E. Eligibility to receive health and dental benefits after 90 days per company handbook. The company pays for 50% of the single premium. If an employee wishes to insure other eligible family members, the employee must cover the additional cost of the insurance. The company reserves the right to increase or decrease their financial obligation for this benefit with 30 days notice.
F. Employees on the performance based compensation plan are not eligible for paid vacation and holidays. Insurance benefits are available after 90 days. Performance based compensation employees are responsible for payment of premiums above 50% of a single premium.
I understand that if I am employed, any misrepresentation or material omission made by me on this application will be sufficient cause for cancellation of this application or immediate discharge from the employer’s service, whenever it is discovered.
The employer does not unlawfully discriminate in employment and no question on this application is used for the purpose of limiting or refusing any applicant from consideration on a basis prohibited by local, state, or federal law.
The application is current for only 60 days. At the conclusion of this time, if I have not heard from the employer and still wish to be considered for employment, it will be necessary to fill out a new application.
If I am hired, I understand that I am free to resign at any time, with or without cause and without prior notice, and the employer reserves the same right to terminate my employment at any time, with or without cause and without prior notice, except as may be required by law. This application does not constitute an agreement or contract for employment for any specified period or definite duration. I understand that no representative of the employer, other than an authorized officer, has the authority to make any assurances to the contrary. I further understand that any such assurances must be in writing and signed by an authorized officer.
