This form is for any high school & middle school student-athlete who is participating in OISD sports. You must re-apply at the beginning of each season (Fall, Winter, Spring). Students who qualify for Free & Reduced Lunch will have their $100 or $150 sports fee waived by OISD. Assistance is provided by the Orcas Island Booster Club and the Orcas Community Resource Center. If you have more than one child participating in sports, please complete one application for each child.
